Description
This pleasant 42-year-old male was involved in an altercation when he was in college and sustained a displaced nose fracture, which he never had repaired. Since that time he has had almost complete obstruction of the left nasal passage. He is unable to breathe through the left side of the nose, and he also snores very loudly. His examination shows a twisted appearance to the nose. He has severe deviation of the septum, which is causing the left nasal obstruction as well as the twisted appearance of the nose. To correct the airway obstruction is necessary that we address not only the deviated septum but also the displaced nasal bones. He underwent rhinoplasty along with a septoplasty under general anesthesia as an outpatient. An open rhinoplasty approach was used. The deviated septum was corrected first. Cartilage from the septum was used to open up the air passages. The nasal bones had to be refracture to get them in the correct position. His operative course was uneventful. The dressing and other bandages were all removed after seven days. He immediately noticed improvement in his left nasal airway, and an improvement in his sense of smell. The twisted nose was corrected as well. His photographs after surgery were taken at six weeks.
